Certified Nursing Assistant (CNA) - Full-Time | Field-Based Vitality Hospice & Palliative Care Vitality Hospice & Palliative Care is a
clinician-owned and operated hospice seeking a compassionate and dependable
Certified Nursing Assistant (CNA) to join our growing interdisciplinary team.
In this field-based role, you will provide hands-on care and emotional support to patients in their homes, helping preserve

Schedule & Position Details - Full-Time, Monday-Friday
- Typical hours: 7:00 AM - 3:00 PM
- Occasional evening/weekend overtime available
- Field-based role with local travel
- Mileage reimbursement provided
- Hiring immediately
Key Responsibilities Under the direction of a Registered Nurse Case Manager, the CNA will:
- Assist patients with mobility, transfers, and ambulation
- Provide personal care including bathing, grooming, dressing, and feeding
- Offer companionship and emotional support to patients and families
- Perform light housekeeping tasks related to patient care
- Accurately document care provided in a timely manner
- Follow individualized care plans and hospice standards
- Support a culture of compassion, respect, and patient-centered care
Qualifications - Active Certified Nursing Assistant (CNA) certification (required)
- Valid driver's license, reliable vehicle, and current auto insurance
- Ability to work independently in the field
- Strong communication, time-management, and organizational skills
- Compassionate, dependable, and patient-focused
- Experience with hospice, palliative care, or chronic illness preferred (not required)
